The Centre intégré de santé et de services sociaux (CISSS) de Laval is currently working to improve access to local health care and social services.

The CISSS de Laval is therefore looking for adults, parents of children under the age of 18, or family caregivers who in the past year have:
  • Received health care or social services at a CLSC in Laval.
  • Used the first-line intake phone services or called Info-Santé/Info-Social.
If you belong to one of these groups, we invite you to take a few minutes to share your experience with us.

This survey should take less than 5 minutes.

The results of this survey will help us determine whether the CISSS de Laval is meeting the changing needs of the population.

The survey consists of 16 questions: 1 open question (comments), 1 multiple-choice question, and 14 questions with the following rating scale:
  • Strongly disagree
