This form is for people who have recently been diagnosed with an STBBI by a health care provider and wish to have their partners notified anonymously by text message.
Have you discussed the various ways you can notify your partners with your healthcare provider? You can ask them if text message is the best option for you and your partners. Texting is an option among other possibilities.
The delay for PEP (post-exposure prophylaxis) will be taken into account if necessary.
The cell phone numbers of partners are required in order to be able to contact them. No other personal information about you or your partner will be collected.
